Community Health and Wellbeing

Community Health and Wellbeing is one of our seven key operational areas. We provide a range of services to help people improve their mental and physical health and wellbeing.

Within our specialist services we recognise that one in four people in the UK will experience a mental health problem each year. That’s why we work together with local authorities, clinical commissioning groups (CCGs), the NHS, local and national charities, and businesses, to help people improve their mental and physical health and wellbeing.

This support means people can better cope with the challenges they face and in turn, function better at home, in work and within their communities living a better quality of life every day.

We do this working in partnership with primary and secondary care organisations, housing providers and a wide array of local support agencies.

This ensures that individuals can get the help and support they need that is right for them to better cope with the challenges they face as individuals.   
We use a number of different approaches to facilitate this support. Our community wellbeing services actively promote and deliver a wide range of social prescription activities which ensure people connect with their community and are supported locally with their mental and physical health needs

Our practitioner support model provides help and advice to more than 15,000 staff and customers every year.

Finally, our partner network in community health and wellbeing actively works with more than 45 local organisations to deliver the right support to people at the time they need it
