Community wellbeing

Our community wellbeing services actively promote and deliver a wide range of social prescription activities which ensure people connect with their community and are supported locally with their mental and physical health needs.

These services help and support people to identify activities within their community that can help them on their journey towards better physical and mental health and wellbeing. With their Shaw Trust Worker they agree a plan that is tailored to their needs, helps engage them in their community and improves their health and wellbeing.

Connect for Health

The Connect for Health service is funded by Ipswich and East Suffolk CCG Transformation Fund and will work across 6 GP surgeries within the Woodbridge community. The service is designed to help individuals who are over 18 years old with a variety of non-medical challenges. Through signposting and support, the Connect for Health service aims to improve health and wellbeing and reduce social isolation.

Connect for Health

Live Well Kent

Live Well Kent is a jointly commissioned service between Kent County Council’s Public Health, Adult Social Care and 7 Clinical Commissioning Groups within Kent.  The service provides a holistic offer of support for individuals living with mental health and wellbeing needs in Kent.

Its aim is to support wellbeing and self management approaches, promote recovery, tackle social isolation and reduce the stigma of Mental Health. At least 1 in 4 people in the UK will experience a mental health problem each year.

At Live Well Kent we help improve mental and physical health and wellbeing. It is a free service for anyone over 17.

You might want to improve your everyday living, become more independent and confident, meet new people, get better skills or find a job. Whatever it is, we’re here to help. We won’t judge you and what we discuss will remain confidential.
